  • I hereby agree to indemnify and hold harmless Caroline Applebee, Raising Rover and the Ontario Veterinary Group, its employees, owners, directors, landlords and agents from any and all claims, suits, injuries, debts, demands, or charges of any nature or kind whatsoever which may be brought by me, or claims by any member of my family or other person accompanying me or my dog to any training class held by Raising Rover in its various training locations. Furthermore, I assume all responsibility for any loss, damage or injury to myself, my guests, my dog or my property, which may have been caused by negligence or any action or inaction, of any person connected in any way with above said classes.

    I certify that my dog is up to date on vaccinations and is friendly towards dogs and people.

    I acknowledge that I have read and fully understand the terms and provisions of this Waiver, Assumption of Risk and Agreement to Hold Harmless and that I freely, voluntarily and unconditionally set my hand hereto.
